About Us

The Holmes County Family & Children First Council is a network of organizations and individuals that promotes safe, healthy, and stable children and families. This is accomplished by communication and facilitating service coordination, setting service priorities, and sharing organizational resources.

Service Coordination

Service coordination is a way that helps bring services to children and families in a simple and organized plan. Families are part of the planning to organize all the services a family needs. All the organizations and agencies a family needs meet at one time to make a written plan.

Four Stages of Service Coordination:

  • 1. Referral to Holmes County FCFC; Assessment of needs; and Identification of the team members that can help.
  • 2. Initial Team Meeting in which the team identifies needs of the family, coordinates existing services and supports, and matches resources with needs. A written plan is developed.
  • 3. Plan implementation/Reviews occur at least every ninety days. Progress toward goals is reviewed and adjustments are made as needed.
  • 4. Transition plans are made as the team nears goal completion.

  • What services does Family & Children First Council provide?

    We identify and address the ongoing needs of children and families through planning, implementing effective strategies, monitoring, reporting and evaluating progress toward achieving desired outcomes.  Holmes County offers family service coordination where all parties meet to access the needs and strengths of the family to develop a strategic plan that involves all the needed agencies to enable the family to meet those needs.

  • Is Family & Children First just another counseling agency?

    Holmes County FCFC is not a counseling agency. The coordinator will help mobilize child and family servicing partners to address the needs of children and families through planning and implementing evidenced-based programs.

  • Who is eligible for service coordination?

    Any family with a child, ages from birth through 24 years old, who is already engaged in more than one system or agency.

  • Will Holmes County FCFC pay for rental assistance?

    No, the council does not pay for rental assistance, but the coordinator can help a family access funding sources if the family participates in service coordination that may help fund this need.

  • Will the Family & Children First Council pay for medical expenses?

    The Family and Children First council does not pay for medical expenses, but if the family participates in service coordination, the coordinator may be able to help the family access funding to pay for medical services.

Members

According to Ohio Revised Code 121.37(B)(1)(a-n), each board of county commissioners must establish a county FCFC. The board may invite any local public or private agency or group that funds, advocates, or provides.
